Insurance Information
  • We are in network with United Healthcare PPO only. All other insurances are out of network. 
  • We accept TriWest/VA with an acupuncture authorization from your VA physician - you must contact your VA physician and have the authorization sent to us first then we will contact you to set up an appointment.
  • We accept Workers Compensation. Once we receive your authorization for treatment, we will contact you to set up an appointment.
  • You may use your HSA or FSA card to pay for your appointments.
  • If you have any other type of insurance, you may still schedule an appointment with us. Full payment will be expected at the time of your appointment (at our cash rate) and we will then provide you with a superbill (within 7 days) which you can submit to your insurance company for reimbursement if applicable.

Insurance Eligibility Verification
 
  • Please submit your United Healthcare insurance information in the Insurance Verification Form, we will get back to you within 2 -3 business days. 
  • Verification of your insurance plan is a courtesy, and can not be guaranteed. Verification of insurance does not guarantee coverage of acupuncture benefits.
